Please complete the species-appropriate form for your pet:

• Canine History Form
• Feline History Form

Submit at least one week prior to your pet’s appointment via email (behavior@cvm.msu.edu) or fax (517-619-1968) prior to your pet’s appointment. Do not mail history forms.

It is also imperative that your pet’s most recent physical examination records be submitted prior to the appointment. Please contact your pet’s family veterinarian and request that they email the full medical history to behavior@cvm.msu.edu, or fax it to 517-619-1968.
