Replacement Shipping Label Request Form

Due to changes made by UPS, return labels now expire four (4) months after they are created. Unfortunately, this impacts many MSU VDL clients who have ordered shipping solutions with UPS delivery. If a label has expired, a UPS driver or staffed drop-off location WILL NOT accept the package. The label will show as invalid in their system.

For Immediate Label Replacement:

If you have a submission ready to send but the label is invalid, please call the Laboratory at 517-353-1683. Our client services team is happy to assist you and can email you a single replacement label for immediate use.

For Replacement of Multiple Labels:

If you have shipping solutions and labels in your clinic that were ordered more than four months ago, they are now invalid. Please complete this online form to request replacement labels. Simply tell us how many expired labels you have, and you will receive your new labels in the mail within 3-5 business days.
