The MSU Veterinary Medical Center's patient release records are managed and controlled by the staff of the Medical Records Service for use in continuing patient care, teaching, and research.
Release of patient information for continuing patient care is an important part of our daily work. Once we receive authorization to transfer records, you may choose to have records sent electronically, via fax, email, or by US mail.
There is not a fee for the release of medical records. However, a fee may be incurred for retrieving the hard-copy records from archive or under certain legal circumstances. There will be a fee for requesting radiography on a disk/USB drive. You may avoid this fee by having radiographs emailed to you instead.
The confidentiality of our patient/client also is very important to us and we will strive to maintain it whenever possible. Under certain circumstances, it may become necessary to provide records to certain local or government agencies without authorization. We also may release records for the continuation of patient care, when your authorization cannot be obtained and when patient care needs are immediate.
