Employee injury
If a MSU Employee (including a student employee) has an accident or is injured at work, the Report of Claimed Occupational Injury or Illness form must be completed (Note – this is a fillable document that may require you to enable macros on your computer): https://www.hr.msu.edu/ua/form...
A copy of this form should be sent to Stephanie Smith-Edwards (Environmental Health & Safety) smiths32@ehs.msu.edu.
In addition, if an employee is seeking medical care for a work-related injury, the Authorization to Invoice MSU form should be completed and taken to the medical provider.

Student injury
Student, Non-Employee
If a student gets injured at MSU or on an off-campus educational experience, the Injury/Property Damage Form must be completed. Please note, this form must be completed if there is a suspicion of a possible zoonotic disease transmission. This form is available on the MSU Risk Management Website.
- The form must be completed by a MSU employee and signed by the employee’s supervisor or an administrator in the Veterinary Medical Center or Veterinary Diagnostic Laboratory.
- The completed form must be sent to Mikaela Berning at berning4@msu.edu.
- Mikaela Berning will send completed form to Risk Management and will document the form in the student’s file, as well as send it to Stephanie Smith-Edwards (Environmental Health & Safety), the Department Head (if the incident occurred on a clinical rotation, class, or lab), and relevant unit administrator.
Student Employee
If a student gets injured while working as a paid university employee, a Report of Claimed Occupational Injury or Illness must be completed https://www.hr.msu.edu/ua/form.... See information listed above under “Employee Injury.”
MSU Visitor/Client Incident
If there is an injury or property damage incident involving a MSU visitor or client, the Injury/Property Damage Form must be completed. This form is available on the MSU Risk Management Website.
- The form must be completed by a MSU employee and signed by the employee’s supervisor or an administrator in the college, Veterinary Medical Center, or Veterinary Diagnostic Laboratory.
- The completed forms must be sent to Risk Management and Insurance riskmgmt@msu.edu; Stephanie Smith-Edwards smiths32@ehs.msu.edu must also be copied, as well as administrators for any other relevant units in the College.
Veterinary Incident Report
If there is an incident involving an animal in the Veterinary Medical Center or Veterinary Diagnostic Laboratory, the College of Veterinary Incident Report Form must be completed. This form is available on the MSU Risk Management Website.
- The form must be completed by a MSU employee and signed by the employee’s supervisor or an administrator in the college, Veterinary Medical Center, or Veterinary Diagnostic Laboratory.
- The completed forms must be sent to Risk Management and Insurance riskmgmt@msu.edu; Stephanie Smith-Edwards smiths32@ehs.msu.edu must also be copied, as well as administrators for any other relevant units in the college. If a student is involved, please also copy Mikaela Berning at berning4@msu.edu.
