• First and Last Name
• Appointment type – Zoom, phone or in-person (if phone, please provide phone number)
• Reason for appointment – Prospective Prevet, DVM or Vet Nursing
• College attending or have attended
• List availability for an appointment
  • Monday Morning or Afternoon
  • Tuesday Morning or Afternoon
  • Wednesday Morning or Afternoon
  • Thursday Morning or Afternoon
  • Friday Morning or Afternoon (Friday is the only day we see in person appointments)
